Sharing your information for Enhanced Access (evenings and weekends)
We are part of the Radford & Mary Potter Primary Care Network (PCN). This means we work closely with other local GP practices and NHS providers to deliver services, including Enhanced Access appointments (evenings and weekends).
To provide you with safe and effective care, authorised healthcare professionals working within the PCN or on behalf of the PCN (such as Nottingham Emergency Medical Service – NEMS) may need to access your GP record.
What information is shared
This may include:
- Your name, date of birth, address and NHS number
- Your medical history and diagnoses
- Medications and allergies
- Test results and letters
- Notes from previous consultations
Why we share your information
We share your information to:
- Provide direct care and treatment
- Ensure clinicians have the full information needed to treat you safely
- Avoid you having to repeat your medical history
- Improve continuity of care across services
How your information is shared
- Information is accessed securely through the NHS clinical system (SystmOne)
- Only authorised staff involved in your care can access your record
- Access is controlled using NHS Smartcards and role-based permissions
- All access is recorded and audited
