You only need a doctor’s note (Fit Note) if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Please contact the surgery via our online access form (ONLINE ACCESS) to request a Fit Note. Please note that if you are requesting your first Fit Note, a GP will need to review you before this will be completed.
